What is NDIS?
Support coordination
“You Decide What Your Vision of a Good Life is, and We Will Help You Achieve It …”
Support Coordination helps to make the best use of the support in your NDIS plan. Support Coordination is a pivotal service offered within the framework of the NDIS Plan, designed to empower participants and optimize the utilisation of their allocated funds and supports.
With a primary focus on capacity building, Support Coordination aims to guide and assist NDIS participants in navigating the complexities of the scheme.
Hence, we have designed our support coordination to deal with service providers, government agencies and regular services. So you do not have to worry about complicated processes.
Our Support Coordinators can:
- Help you find the right supports to live the life you want.
- Explain the NDIS plan to you so that you can use the supports in your plan to live a better life.
- Help you in defining what a good life means for you and how you can work towards it
- Help you to identify the support you need to work towards the goals outlined in your NDIS Plan.
- Provide information and support to help you choose the service providers you prefer.
- Help you understand the idea of "choice and control" and what to do if things don't work in your favour.
- Connect you with your local community so that you can gain access to its resources.
- Develop your skills, capacity, and resilience.
- Support you navigate crisis situations.
- Help you prepare for your NDIS annual review.
Having an NDIS plan can be overwhelming. But having choices is also crucial to ensure you get the support you need, when you need it. We understand that organising supports can be difficult for you.
